All steps for enrollment are listed here, and all required documents are listed below for your convenience.
Required Forms/Document:
1) Application Form
2) Current Immunization Record (CA yellow card)
3) Photo identification
4) Proof of residency – Current utility bill (SMUD, PG&E, or water) with correct name and address
5) Current withdrawal grades (last report card from previous school) or transcripts
6) Proof of Birth - Copy of student’s birth certificate
7) Guardianship/Custody papers (if applicable)
